Education Transcripts
In order to make transcripts more accessible to Creighton University Medical School Faculty, and to assist in acquiring Continuing Medical Education (CME) information needed for licenser or other reports, the Continuing Medical Education Division has developed a program whereby you can have your education transcript printed in your department.
The education transcripts list the CME programs you have attended and include the program name, date, and number of education credits received. Credit is based on your signature, therefore, it is imperative that you sign in at each Creighton sponsored conference or grand rounds for which you desire credit.
Contact the person listed below in the department where you have your primary faculty appointment. Please provide the contact person with your social security number (used for record processing only) and the time frame for which for which you need the program information.
If your primary faculty appointment is NOT one of the following departments, contact Lee Taylor in the Continuing Medical Education Division at 280-1830 for your transcript.
